Invoice Request Form –
BASIC Support Package ($797 AUD)

Please complete the form below. Once submitted, your personalised invoice with all legal requirements for NDIS will be automatically generated and sent to your email. You will then forward this to your plan manager, who can arrange payment. Once payment is processed, you will automatically receive access to the course and the Re-MIND General Community Group.

To complete this form, we recommend following these steps:

Step 1: Sign into the NDIS Portal (MyPlace) via MyGov

Step 2: At the top right where it says "Acting As", select your child's name.

Step 3: Click on the box that says [Child's Name] Plan.

Step 4: Click on the box that says "View [Child's Name's] Plan.

Step 5: Get the NDIS Participant Number and type it or paste it in the relevant field below.

Step 6: Click on the drop down that says [Child's Name's] Goals. Type or copy and paste any relevant goals relating to emotional or relational regulation, social skills, etc into the box below.

Step 7: Click on the drop down that says Funded Supports Information, and then select an approved funded support category from the drop down below.
